IndexGofer can help
IndexGofer shows you a page of text, its index entries (in yellow), and a list of all possible index entries. One double click adds an entry to a page. When you are done, IndexGofer formats the index from your entries.

Essential to a good index is a good set of terms. With IndexGofer you edit the terms list in a tabular display, as at the right. Trigger phrases are in the left column while terms, sub terms, and reference terms are to the right.

To the right is a bit from the index generated for Hansen, Susan B., Religion and Reaction (Rowman & Littlefield, 2011)
